PAYMENT POLICIES

Payment Policy We accept cash, check, Visa, MasterCard, American Express, and Discover cards.
We can also accept payment via HSA's and Flex-Spending Accounts.We work with Personal Injury and Worker's Compensation cases in our private practice, within the State of Wisconsin.
Cases may vary, check with your insurance prior to scheduling your first appointment.

Payment is due when services are rendered.  Same day payment is expected only when not billing insurance. We can provide insurance billing, on a case by case basis. Please contact us before your appointment
​to verify coverage and/or co-pay. If you are self submitting,
we can provide a superbill for submission to insurance for reimbursement upon our patients' request.

CANCELLATION POLICY

Appointments must be canceled 24 hours in advance to allow us to schedule other patients. Appointments canceled less than 24 hours in advance will be billed full charge. Exceptions will be made at our discretion in the
event of inclement weather or real emergencies.
